How to Request Records
To request records
In order to request your records:

Call or email your provider and let them know what records you are requesting and who you would like them released to:

If the records are to be released to a third party, we will provide you with a Release of Information form that will need to be completed and signed.
Processing timeline
Unless otherwise agreed to be necessary, I retain clinical records only as long as is mandated by law. As a client, you have the right to review or receive a summary of your records at any time, except in limited legal or emergency circumstances or when I assess that releasing such information might be harmful in any way. Considering all of the above exclusions, if it is still appropriate, and upon your request, I will release information to any agency/person you specify in writing. Per LPC Board Rules, Counselor will provide copies or a summary of the materials within 15 days of the request. When more than one client is involved in treatment, such as in cases of couple and family therapy, I will release records only with signed authorizations from all the adults (or all those who legally can authorize such a release) involved in the treatment.

Fees
Copies of the Client’s Record: The Client will pay thirty-five cents ($0.35) per page that is photocopied or printed from his/her chart in addition to a five dollar ($5) service fee for copying 1-15 pages, ten dollars ($10) for 16-35 pages, fifteen dollar ($15) for 36-55, twenty dollars ($20) for 56-80, twenty-five dollars ($25) for 80--105 pages, and thirty dollars ($30) for 106-150 pages for time spent preparing the record. If the records are subpoenaed or requested, the client will be expected to pay for the cost of photocopying the records if those costs are not paid by the party issuing or requesting the copies. The fee will not apply to photocopies being sent to another mental health or medical professional, such as a family doctor, psychiatrist, or neurologist. If the records are being mailed, then a fee will be assessed for the cost of mailing the items using certified mail with a return receipt. If a document needs to be notarized, then a $10 fee will be assessed.
